How To Submit Your Product Warranty Claim:

Please fill out the Warranty Claim submission form below in order to begin processing your warranty claim. Once your information is submitted, a Warranty Claim Representative will be in contact with you for a few additional details.

Download PDF of Warranty Claim form here. Fill in the Warranty Claim information and submit either by mail or by fax. Once your information is received, a Warranty Claim Representative will be in contact with you for a few additional details.

NOTE: If your product is deemed warrantable, you may be asked to print, fill out, & attach a physical copy of this form, with the returned product in order to help expedite the warranty process.

PHOTOS: A picture always helps. Please include any reference pictures (if necessary), of your warrantable InnoMax sleep product in order to help expedite your warranty claim.
